Full Job Description
At Home Solution seeks a full-time Case/Client coordinator to join our progressive team. The successful candidate possesses strong decision-making skills, excellent communication, and time management skills in this role. The coordinator must work directly with patients and caregivers to support the daily operation of the agency. Bilingual Russian/English is a must.
Responsibilities:
- Schedules/Assigns caregivers to patient cases
- Interact with families, clients, and caregivers regarding services
- Follow-up with clients and caregivers regarding coverage, call-offs, and cancellation of staff.
- Maintain open and current communication with the management team regarding changes in the status of clients and caregivers.
- Actively participate in regular staff meetings to provide guidance on any changes with staffing requirements/needs.
- Documents all notes and updates for patients and caregivers into HHA Exchange
- Participates in agency on-call when scheduled
- Ensures supervision and direction to HHA personnel
- Receives referrals and inquiries for services
- Reports all incidents to the immediate supervisor
- Ensure that services are provided as prescribed by the client’s Plan of Care.
- Prebilling and working on Call Dashboard on a daily basis
- Performs other duties as needed
Requirements:
- HHA Exchange knowledge preferred
- Prior experience is a plus, but willing to train the right candidate
- Coordinating/Case Management: 1 year (Preferred)
- Be an organized and detail-oriented person
- Excellent communication skills
- Must be able to read, write and understand English
- Ability to work independently and make decisions
- Ability to promote staff motivation
- Ability to multitask
- Proficient in Microsoft Office (Outlook, Word, Excel)
- Must have excellent listening, written, and oral communication skills.
- Working under time pressures and managing multiple activities at one time is essential.
- COVID-19 vaccination required.
Job Type: Full-time
Pay: $50,000.00 – $55,000.00 per year
