TITLE: Spanish/Russian Speaking-Care Navigator/Outreach Specialist
REPORTS TO: Outreach Director
JOB SUMMARY
Interborough Developmental and Consultation Center is seeking a care navigator/outreach specialist. The care navigator/ outreach specialist will provide face-to-face, telephonic, and written outreach to engage and enroll new patients.

DUTIES:
- Provide comprehensive education to potential Health Home clients that highlight the benefits and promotes participation in care coordination services.
- Responsible for providing progressing diligent search efforts that includes mailing letters, making phone calls, and community/home visits Responsible for managing an assignment list potential Children/Family Health Home members over a 30 day period
- Track and document all outreach attempts and enrollment as required.
- Screen clients for eligible for enrollment into program.
- Outreach Specialist is expected to meet organization enrollment goals.
- Utilize internet databases, health record systems, and other resources to explore client eligibility and contact information.
- This position requires the candidate to be on the field about 75% of the time and take mass transit to travel from one site location to another.
- Track the outreach and engagement of all Lead Health Home referrals as required.
- Gather, review and file medical, mental health & substance use reports from respective providers as needed.
- Create calling and field work schedule on a weekly basis.
- Achieve minimum 15% – 20% enrollment based on monthly individual caseload. .
- Update and maintain accurate records on prospective clients of services rendered on required software platforms.
- Actively participate in any organizational activities (meetings, events, etc.).
- Represent the agency at various venues to promote agency services.
- Responsible for maintaining and tracking the progressive of outreaching those members they are assignment to and their outcomes.
- Responsible for engaging community referrals (ground ups) to educate them on the benefits of care coordination services and have them sign consent forms for service.
- Responsible for establishing relationships that are necessary for the continued engagement and care coordination services within the community.
- Perform various clerical duties: Filing, run Medicaid checks, assemble and disassemble charts, develop client contact sheet, update client caseload, complete team weekly reporting form at weekly team meetings, assist the team in compiling information for the monthly report, etc.
- Conduct assigned administrative duties (mail, follow-up etc.) and other assigned job duties.
Qualification
- Bilingual in SPANISH OR RUSSIAN A PLUS
- Bachelor’s Degree in Social Work, Human Services, Psychology or related field
- 2 years of full-time work experience in community outreach, involving public speaking, meeting with individuals and/or groups, ideally in an agency involving services to children and families
