Home Care Coordinator (Russian Speaking)

Five Star Home Health Care Agency is the one of the top home care agencies in NYC, servicing Brooklyn, Staten Island, Manhattan, Queens and Bronx. We are currently looking for a Russian/ English speaking Coordinator to work a full time position.

Qualifications For Position

• must speak Fluent Russian and English
• Multi Year experience in Home Care industry
• HHAExchange knowledge is a plus
• organized and detail oriented
• Excellent communication skills
• Must be able to read, write and understand English
• Ability to work independently and make decisions
• Ability to multitask

Responsibilities

• Responsible for scheduling and assigning Home Health Aide to meet patient needs
• Ensure all Patients are receiving services
• Ensure that services are provided as prescribed by the client's Plan of Care.
• Manage Schedules of Home Health Aides and Patients.
• Document all changes and occurrences for Patients and Aides on a daily basis
• Communicate with patients and aides to ensure they satisfied with care provided
• Keep tabs health status with the patient through bi-weekly progress calls
• Prebilling and working on Call Dashboard on a daily basis
