Intake Coordinator – Home Care Russian Speaking

INTAKE COORDINATOR

Licensed Home Care Service Agency servicing the five boroughs of NYC is seeking to hire a seasoned Intake Coordinator.

Location of position Forest Hills, NY

Must have at least one year of “home care” experience

Must Speak Russian

Responsibilities and Duties:

• Assess patient referrals to assure they are appropriate for those services provided by Tri-Med Home Care Services, Inc.
• Communicate with all relevant parties (i.e. patient, referral source & agency staff) to expedite the processing of patient referrals.
• Assure the accuracy of patient/referral information (i.e. patient information, demographics, referral source & other agency mandates).
• Assist with the tracking of all referrals to assure appropriate follow up from inception of referral to admission.
• Respond to patient and agency requests for additional services or transfer of services to another insurance plan.
• Assist with the development of new referrals in conjunction with agency MLTCP plan business development initiatives.
• Assist with obtaining MLTCP/MCO authorizations for services.

• The ideal candidate for this position will possess excellent communication/interpersonal skills
• Able to coordinate multiple activities concurrently
• Good organizational skills
• Good time management skills

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
