Full Job Description
New York State Early Intervention is a program for infants and toddlers under the age of three with suspected developmental delays or disabilities. Service Coordinators are at the forefront guiding and advocating for enrolled families to ensure a smooth process for their child; from the evaluation stage and throughout service delivery. The Early Intervention Service Coordinator will help families identify and prioritize concerns while adhering to the timelines and procedures set forth by the Department of Health and Mental Hygiene.
Responsibilities:
Conduct periodic community visits
Oversee and manage your own child caseload (with ongoing supervision)
Act as a liaison between families and providers
Develop strategies to meet the family needs
Assist families in gaining access to services and benefits that may be available to them
Assist family with community resources
Attend meetings to assist in development of child service plans (virtual meetings available)
Ensure child plans are followed as authorized by the City
Guide family in transitioning to other programs such as Committee on Preschool Education (CPSE)
Timely document and enter coordination activities with families into an electronic database (comprehensive training provided)
Education:
Bachelor’s Degree in health or human services (ex: BS/BA Psychology, Sociology, Education, Nursing, Social Work, etc.)
***HOLDERS OF INTERNATIONAL/FOREIGN EQUIVALENT DIPLOMAS IN EDUCATION, HEALTH & HUMAN SERVICES, PSYCHOLOGY, SOCIOLOGY ARE WELCOME TO APPLY***
Required Skills:
Proficient in typing and navigating electronic databases,
Excellent time management and organizational skills,
Demonstrated ability to multi-task and meet deadlines.
Spoken knowledge of one of the listed Foreign Languages (Russian, Uzbek, Urdu, Tajik) is required.
Salary: $50,000. Potential to earn $60,000+ benefits within 3 months.

Experience:
Prior early Intervention experience is not required, but is a plus.
